Below you will find both forms needed at ALL of your Hero Sessions for Inspiration Through Art
Email inspirationthroughart@gmail.com to download the complete folder with all the forms, flyers, and brochures!
-Must be signed by the parents at the time of each hero session you conduct. Please keep all of your release forms and photos together after each session. If asked, we may need the original copy(s) sent to us for publication use. We also ask that this form be scanned and sent to us when you send in your digital photographs! This can be done on the CONTACT us page!
-Print and take this document with you for each hero session. This is our small way of thanking them allowing us to be a part of their hero’s journey!
